Pleine conscience et stress chez les parents d'enfants avec un diagnostic de "troubles du spectre de l'autisme" en Chine

Être parent d'un enfant atteint d'un trouble du spectre autistique (TSA) peut être extrêmement difficile. Il est important d'identifier les facteurs associés au stress parental. 

Cette étude a examiné l'effet indirect de la pleine conscience dispositionnelle des parents sur leur anxiété et leurs symptômes dépressifs et la qualité de vie de la famille (FQOL) par le biais de la parentalité consciente, puis du stress parental. 

Soixante-dix-neuf parents chinois (24,1% de pères) d'enfants avec un dTSA âgés de 3 à 13 ans ont rempli des questionnaires d'auto-évaluation. 

Les résultats ont indiqué qu'une plus grande vigilance dispositionnelle était associée à une plus grande vigilance parentale, qui était liée à un stress parental plus faible, et en outre liée à une moindre anxiété et des symptômes dépressifs et à une FQOL plus élevée. 

Les résultats fournissent des informations précieuses sur les voies potentielles par lesquelles la pleine conscience et la parentalité consciente peuvent avoir un impact positif sur le bilan des parents.

Mindfulness and Stress Among Parents of Children with Autism Spectrum Disorder in China

Abstract

Parenting a child with autism spectrum disorder (ASD) can be tremendously challenging. It is important to identify factors associated with parenting stress. This study examined the indirect effect of parental dispositional mindfulness on their anxiety and depressive symptoms and family quality of life (FQOL) through mindful parenting and then parenting stress. Seventy-nine Chinese parents (24.1% fathers) of children with ASD aged 3-13 years completed self-report questionnaires. Results indicated that higher dispositional mindfulness was associated with higher mindful parenting, which was related to lower parenting stress, and further related to lower anxiety and depressive symptoms and higher FQOL. The findings provide valuable insight into the potential pathways through which general mindfulness and mindful parenting may positively impact parental outcomes.

La pleine conscience pour les enfants et les adultes avec un diagnostic de "troubles du spectre de l'autisme" et leurs soignants: une méta-analyse

Les thérapies basées sur la conscience gagnent en popularité. Cependant, les preuves de leur efficacité pour réduire la détresse psychologique et améliorer le bien-être des familles vivant avec des "troubles du spectre de l'autisme" (TSA) sont limitées. Une recherche systématique a identifié 10 études indépendantes portant sur un échantillon groupé de 233 enfants et adultes avec un diagnostic de TSA (dTSA) et sur 241 soignants. Les tailles d'effet g des couvertures avec des intervalles de confiance associés de 95%, en plus de l'hétérogénéité, ont été calculées à l'aide d'un modèle à effet aléatoire. Les aidants naturels, les enfants et les adultes ayant reçu un état de conscience complet ont tous signalé des gains importants en termes de bien-être subjectif immédiatement après l'intervention. Les données disponibles indiquent que les effets de l'intervention ont été maintenus à 3 mois de suivi. La pleine conscience présente une stratégie d’intervention prometteuse dans les populations de TSA, mais des recherches plus contrôlées sont nécessaires pour déterminer son efficacité avec précision pour les familles et les sous-groupes touchés.

CONCLUSION
Les interventions axées sur la pleine conscience semblent atténuer un certain nombre de problèmes de santé interpersonnels et mentaux ayant une incidence sur le bien-être subjectif  chez les enfants et les adultes avec un dTSA et sur leurs soignants.
Des recherches quantitatives supplémentaires sont nécessaires pour confirmer les effets positifs rapportés dans cette revue, en plus de la possibilité de généraliser ces résultats sur une population plus large de la population de TSA. Cela inclut les futurs essais contrôlés pour aider à prendre en compte les effets de l'intervention, en plus d'explorer les différences potentielles entre les sexes dans la réponse au traitement.

Abstract

Mindfulness-based therapies are rising in popularity. However, evidence for their effectiveness in reducing psychological distress and enhancing wellbeing for families living with autism spectrum disorder (ASD) is limited. A systematic search identified 10 independent studies, involving a pooled sample of 233 children and adults with ASD and 241 caregivers. Hedges' g effect sizes with associated 95% confidence intervals, in addition to heterogeneity, were calculated using a random-effects model. Caregivers, children and adults who received mindfulness all reported significant gains in subjective wellbeing immediately post-intervention. Available data indicated intervention effects were maintained at 3-month follow-up. Mindfulness presents a promising intervention strategy in ASD populations, however more controlled research is required to determine its precise efficacy for affected families and subgroups.

Les thérapies psycho-corporelles sont souvent utilisées par les personnes avec un diagnostic de trouble du spectre de l'autisme.
PubMed, PsychInfo et Scopus ont été utilisés pour des recherches avec les TSA et les thérapies corps-esprit.
Seize études ont été sélectionnées pour examen; Ces études ont testé les interventions en utilisant la vigilance, la méditation, le yoga, Nei Yang Gong et la thérapie d'engagement d'acceptation.  
La plupart des résultats d'étude ont ciblé les comportements, les symptômes psychologiques et la qualité de vie pour les enfants et les adultes avec un diagnostic de TSA ainsi que leurs parents. 
Seulement trois des études étaient des essais contrôlés randomisés.  
Les preuves de l'intérêt des thérapies psycho-corporelles pour les personnes avec un diagnostic dce TSA sont limitées et bénéficieront d'essais contrôlés randomisés plus larges.

Autism and Mind-Body Therapies: A Systematic Review

Abstract

BACKGROUND:

Mind-body therapies are often used by people with autism spectrum disorders (ASD). However, there has been little examination into which types of mind-body therapies have been investigated for people with ASD and for what purposes. A systematic review was conducted to evaluate the existing evidence for mind-body therapies for people with ASD, particularly to determine the types of mind-body therapies used and the outcomes that are targeted.

METHODS:

PubMed, PsychInfo, and Scopus were searched using terms for ASD and mind-body therapies. Sixteen studies were selected for review; these studies tested interventions using mindfulness, meditation, yoga, Nei Yang Gong, and acceptance commitment therapy. Most study outcomes targeted behavior, psychological symptoms, and quality of life for children and adults with ASD as well as their parents.

RESULTS:

There was little overlap between studies on the types of mind-body therapies used and associated outcomes, and only three of the studies were randomized controlled trials. Most studies were small and uncontrolled. Some studies modified the mind-body therapies to increase accessibility for people with ASD.

CONCLUSION:

The evidence for mind-body therapies for people with ASD is limited and would benefit from larger randomized controlled trials.

Abstract

BACKGROUND:

Despite the dramatic increase in autism spectrum disorder in youth and the extremely high costs, hardly any evidence-based interventions are available. The aim of this study is to examine the effects of mindfulness training for adolescents with autism spectrum disorder, combined with Mindful Parenting training.
Malgré l'augmentation spectaculaire des troubles du spectre de l'autisme chez les jeunes et les coûts extrêmement élevés, pratiquement aucune des interventions fondées sur des preuves n'est disponible. Le but de cette étude est d'examiner les effets de l'entraînement à la pleine conscience pour les adolescents avec des troubles du spectre autistique, combinée à une formation l'attention parentale. 

METHOD:

A total of 23 adolescents with autism spectrum disorder, referred to a mental health clinic, received nine weekly sessions of mindfulness training in group format. Their parents (18 mothers, 11 fathers) participated in parallel Mindful Parenting training. A pre-test, post-test, and 9-week follow-up design was used. Data were analyzed using multi-level analyses.

RESULTS:

Attendance rate was 88% for adolescents and fathers and 86% for mothers. Adolescents reported an increase in quality of life and a decrease in rumination, but no changes in worry, autism spectrum disorder core symptoms, or mindful awareness. Although parents reported no change in adolescent's autism spectrum disorder core symptoms, they reported improved social responsiveness, social communication, social cognition, preoccupations, and social motivation. About themselves, parents reported improvement in general as well as in parental mindfulness. They reported improved competence in parenting, overall parenting styles, more specifically a less lax, verbose parenting style, and an increased quality of life.
Le taux de participation était de 88% chez les adolescents et les pères et 86% pour les mères. Les adolescents ont signalé une augmentation de la qualité de vie et une diminution de la rumination, mais aucun changement dans l'inquiétude, les principaux symptômes de troubles du spectre autistique, ou la pleine conscience. Bien que les parents n'aient signalé aucun changement dans les principaux symptômes du trouble du spectre autistique des adolescents, ils ont signalé une amélioration de la réactivité sociale, de la communication sociale, de la cognition sociale, des préoccupations et de la motivation sociale. Sur eux-mêmes, les parents ont rapporté une amélioration en général ainsi que dans l'attention des parents. Ils ont signalé l'amélioration des compétences de la parentalité, des styles parentaux en général, plus spécifiquement un style parental moins laxiste et moins verbal et une meilleure qualité de vie. 

DISCUSSION:

Mindfulness training for adolescents with autism spectrum disorder combined with Mindful Parenting is feasible. Although the sample size was small and no control group was included, the first outcomes of this innovative training are positive.
La formation à la pleine conscience pour les adolescents avec des troubles du spectre autistique combiné avec l'attention parentale est possible. Bien que la taille de l'échantillon était petit et qu'aucun groupe témoin n'ai été inclus, les premiers résultats de cette formation innovante sont positifs.
